Please do not come to our office if you answer yes to any
of the following questions:
Have you traveled outside of the country in the last 14 days?
Do you have a temperature; shortness of breath; dry cough; difficulty breathing?
Have you been exposed to someone that has been tested and was positive or waiting for test results?
Are you experiencing a sudden onset of extreme fatigue?
Have you suddenly lost your sense of taste or smell?
If you answer yes to any of these questions, please call our office first!
